Low dose combined spinal and epidural anesthesia in a parturient with severe mitral stenosis and severe pulmonary arterial hypertension for cesarean section

We describe the anesthetic management for an elective cesarean section in a parturient with severe mitral stenosis and severe pulmonary arterial hypertension using a low dose of 5 mg hyperbaric 0.5% bupivacaine and 25 mcg fentanyl for spinal block supplemented with epidural lignocaine to achieve adequate level. This patient was vulnerable to develop complications due to hypotension and tachycardia which are usual complication of neuraxial blockade. This case reports highlights the hemodynamic stability using this anesthetic modality in a severe mitral stenosis parturient.
